Smith County Board of Education 12/20/05 Review: Annually, Medicines 02/17/04
The Smith County School policy for medication dispensing at school is as follows:
1. If the medication is a prescription, the physician must fi ll out the Physician Form for
Medication. Both the physician and parent/legal guardian must sign the form. A new
Physician Form is required each school year.
2. If an over-the-counter medication is necessary at school, the parent/legal guardian must
fi ll out and sign an Over-the-Counter Medication Form.
3. A Release of Information Form needs to be signed by the parent/legal guardian for all
4. The parent/legal guardian or responsible adult must bring the medication to school. Pre-
scription medication must be in a properly labeled pharmacy bottle, which the pharmacist
will provide. The label on the medication bottle must match the physician's order. The
empty medication bottle will be sent home with the student, but the parent/legal guard-
ian is required to bring the medication to school. Over-the-counter medication i.e. cough
drops, Tylenol, Midol, Benadryl, creams, etc. must be brought to school by the parent
in the original unopened container with the student's name written on the bottle. Calls

5. Herbal Supplements and over-the-counter medications, when taken on a daily basis, must
have the Physician Form for Medication fi lled out by a physician. The medicine must be
brought to school by parent/legal guardian in the original unopened container with the
student's name written on the bottle.
6. All medication will be kept in a secure, locked area except: inhalers, epi-pens, glucom-
eters and insulin, which are allowed to stay with the student after the Physician Form and
Release of Information is fi lled out and brought to school.

8. Emergency Glucagon Administration-School personnel who volunteer under no duress or
pressure and who have been properly trained by a registered nurse employed or contracted
by the Smith County Board of Education may administer glucagon in emergency situa-
tions based on that student's Individual Health Plan (IHP). However, if a school nurse is
available and on site, the nurse shall provide this service to the student. The school nurse
employed or contracted by the Smith County Board of Education shall be responsible
for updating and maintaining each IHP. The current "Guidelines for Use of Health Care
Professionals and Health Procedures in a School Setting" shall be used as a guide by the
registered nurse in training volunteer school personnel to administer glucagon. Training
to administer glucagon shall be repeated annually and competencies documented in the
employee's personnel fi le. The provisions of TCA 49-5-415 subdivision (a)(3) regarding
protection from liability shall apply also to the volunteers who provide services pursuant
to this subsection (b) and the registered nurses who provide their training. Medicines 6.405

for emergency epinephrine injections which allows any layperson who volunteer under no
duress or pressure and who have been properly trained by a registered nurse employed or
contracted by the Smith County Board of Education may administer an epinephrine injec-
tion in emergency situations based on that student's Individual Health Care Plan (IHP).
The school nurse employed or contracted by the Smith County Board of Education is
